La Vie’s Point Direct Program
Point Direct is specially designed for individuals who prioritize continuity of care but prefer not to undergo the Comprehensive Wellness Assessment at the onset of their relationship with La Vie. We recognize that some individuals may have their reasons for opting out of the initial Comprehensive Wellness Assessment, yet still desire ongoing support and access to quality healthcare services.
Program Overview
Point Direct mirrors many of the benefits and features of our Point One Care program, ensuring that members receive comprehensive, year-round healthcare support. The program is designed to provide seamless continuity of care without the requirement for the initial Comprehensive Wellness Assessment.
Virtual Care Program
Members enrolled in Point Direct will have access to our Comprehensive virtual care program, allowing them to connect with healthcare professionals remotely for consultations, follow-ups, and medical advice.
Dedicated Nurse Case Coordinator
Similar to Point One Care, Point Direct members will have access to a dedicated Nurse case Coordinator who will serve as their primary point of contact, providing personalized guidance and support throughout their healthcare journey.
Medical Referrals and Follow-Ups
Included in the services are all necessary medical referrals and appointment follow-ups ensuring that members have access to healthcare providers when needed.
Year-Round Care
Point Direct provides continuous care throughout the year, ensuring that members receive support and assistance whenever necessary.
